This scope of work proposal will outline our facility needs and requirements for GULDMANN Position Combi Locks to be added to our in-patient areas as follows: (11 Position Locks)



GULDMANN and/or Certified Technician shall provide technical and cost proposal to ensure turn-key services to install GULDMANN Position Combi Locks to our current GULDMANN patient lift systems, listed above, in accordance with this scope of work.



This cost proposal shall include but not limited to; OEM GULDMANN Position Combi Locks system, support materials, bracing hardware and all other associated components required for complete installation, supporting hardware for all current in-patient GULDMANN patient ceiling lift systems, listed above to include but not limited to; GULDMANN Position Combi Locks system, support material, bracing and all other associated components required for complete installation of GULDMANN Position Combi Locks to our existing GULDMANN patient GH3 ceiling lifts and include install details, documents, tools, labor, testing, dust containment systems, direct care staff training, cleaning supplies, etc. GULDMANN Certified Technicians will ensure each lift system has smooth function, without fast or jerky starts or stops and not affect the function of our current GULDMANN patient ceiling lift systems operation.



Requirements:




  1. Vendor will ensure technician will not perform any work/preventive maintenance inspections in an area/room occupied by a Veteran or Staff member.

  2. Vendor will be required to perform all work during the hospital normal business hours (M-F 8:00 AM – 4:30 PM) excluding federal holidays.

  3. Vendor will provide all travel, labor, parts, test equipment (to include weight set), tools, and incidentals necessary to perform all required inspection services.

  4. Vendor will ensure GULDMANN Certified Technician, report any lift installation that modifies the function of a room’s privacy curtains to POC and/or Designee with input from Manager and/or Designee. The GULDMANN Certified Technician will submit mitigation plan to POC and/or Designee with input from Manager and/or Designee for review and approval.

  5. Vendor will ensure all services for this contract meet manufacturer's performance and technical specifications, Federal Regulations, and other specifications that may apply such as National Electric Code (NEC), Life Safety Code (LSC), Joint Commission (JC), and VA Regulations. There is a potential for exposure to blood borne or other infectious material with equipment throughout the hospital.

  6. Vendor will ensure GULDMANN Certified Technicians install Position Combi Locks approved for use with GULDMANN Ceiling Lift systems.

  7. Vendor will ensure GULDMANN Certified Technician will complete all required reports to include but not limited to: GULDMANN installation documents and completion of all sections of VHA Installation or Relocation Checklist for Ceiling Mounted Patient Lifts, when any motor/hoist is removed from the patient lift rail.

    1. VHA Installation or Relocation Checklist for Ceiling Mounted Patient Lifts include:

      1. Motor/Hoist information: Serial Number, EE# assigned by VHA, location.

      2. Pre-Installation

      3. Installation

      4. Post-Installation

      5. Rails/Tracks and End Stops

      6. Lift Unit and Straps

      7. Load Testing

      8. Manuals

      9. Training

      10. Sign to validate all of the above sections were completed as required





  8. Vendor will ensure GULDMANN Certified Technician shall ensure the designated BioMed/Engineer staff are notified and/or present for weight testing of ceiling lift systems and all weight tests are completed prior to VHA acceptance.

  9. Vendor will ensure GULDMANN Certified Technician shall provide the following to be considered for award: 1.Provide a specific timeline for installation. 2. A three-year warranty on equipment. 3.Provide Plan of Equipment Certification after install. 4.Shall submit enough product information to allow the Government to evaluate the product(s) being offered.
